Medical & Surgical Clinic of Irving Successfully Implements Chronic Care Management Program

Medical & Surgical Clinic of Irving now offers a Chronic Care Management program for Medicare patients with chronic conditions. This allows the Seniors to receive additional resources and allows M&SCI to better track their care.

Medical & Surgical Clinic of Irving (M&SCI) is proud to announce that on March 1st, they brought live the Chronic Care Management or “CCM” program in partnership with Allscripts. Effectively managing the care of Medicare patients with chronic conditions takes more than just an occasional face-to-face office visit. Seniors need and deserve outreach, telephone conversations, medication reconciliation and coordination among caregivers, along with other activities. And, that’s just what M&SCI does to ensure the best care for their patients.

CCM allows M&SCI to better track these activities and provides additional resources for their patients. Enrolled patients will now have 24/7 access to care services by telephone. And, the clinicians providing after-hours care will have full access to the patient’s plan of care and medications through the system. In addition to the care patients are already receiving, clinicians conduct a monthly medication reconciliation to audit the medications the patient is taking. This adds an additional layer of safety, ensuring the patients are on the proper doses of the right medications and that drug interactions are minimized.

Medical & Surgical Clinic of Irving is dedicated to the health of their community and providing value-based care. “The goal of CCM is to provide Medicare patients with a customized care plan and regular patient engagement through a Care Manager in order to achieve a healthier patient population. The expected outcomes of reduced ER visits and unnecessary hospital stays or readmissions; better control of chronic conditions like diabetes and high blood pressure and improved medication monitoring will ultimately help reduce the cost of healthcare.” says Jeffry Michael Duke, CPA, Executive Director of Medical & Surgical Clinic of Irving.

The program was successfully implemented after months in development. M&SCI enrolled nearly 75 patients the first week and the feedback has been extremely positive. Additional eligible patients are now being contacted regarding their chronic care management. M&SCI expects to enroll more than 2,500 Medicare and Medicare Advantage patients by August 31, 2016 to offer the highest level of care possible to these chronically ill patients. M&SCI is dedicated to ensuring the best care for all of their patients. This Chronic Care Management program is one more vital service they offer to reach this goal.

Introducing MSCI’s Kidney Stone Clinic

Kidney stones are no fun. Unfortunately, research indicates that the rate of kidney stones in the US is actually on the rise: currently 13% of men and 7% of women will develop kidney stones at some point in their life. Even more alarming, 50% of individuals who develop kidney stones will have another kidney stone within 10 years. Finally, there’s the cost: Americans will spend more than $5 billion on kidney related treatments and diagnoses. MSCI is here to help those suffering from kidney stones and those at risk of developing kidney stones by introducing our kidney stone clinic.

Kidney stones can cause pain if they become too large. At a certain point, they begin to obstruct the drainage system, which can cause damage to the kidneys. Over time, this might even lead to kidney infection, bleeding, or kidney failure. Under certain circumstances, you might need to undergo surgery.

There are many factors that can cause kidney stones, but some of the most common factors include concentrated urine, high urinary calcium, uric acid, and substances that inhibit urinary functions like citrate. Because many of these factors are from lifestyle choices, some of these conditions may persist after a kidney stone, hence the likelihood that a reoccurrence takes place within 10 years

Our kidney stone clinic offers help to those suffering from this disease. Combining the expertise of our nephrology and urology departments, our interdisciplinary clinic helps explain the risk factors of kidney stones to patients while also treating existing kidney stones. Dr. Grandhi and Dr. Smith work together to provide comprehensive care to patients, creating individualized care plans that help minimize the chance of reoccurrence.

The kidney stone clinic at MSCI offers treatments for:

-Multiple kidney stones

-Counseling for those with a family history of kidney stones

-Patients who need kidney stone removal

-Stones that are comprised of various acids

Kidney stone pain can be excruciating, so if you are worried that you have a kidney stone, be sure to talk to a physician as soon as possible, particularly if you have a family history of kidney stones. Our clinic in Irving offers the latest treatments and exceptional doctors who will do their best to put you on the road to recovery. Dr. Edelman and His Book Featured on Fox 4

The over-usage of sugar in our society is a serious problem, and Dr. Edelman is trying to spread the word about this oversaturation of the sweet stuff. Recently, Dr. Edelman went on Fox 4 to talk about his book Sugar Rehab and the dangers of consuming too much sugar.

Watch Dr. Edelman on Fox 4 here:

Sugar Rehab utilizes the latest research to show how people consume more sugar than they actually realize. Many seemingly innocent foods, like bread and even fruit, can contain a surprisingly large amount of sugar, which can easily disrupt diets and affect your body’s natural equilibrium.

The book does not try to be a dieting guide but rather as a handbook to reevaluate your lifestyle and improve your overall living by making you more conscious of the decisions you make related to sugar-loaded foods. In fact, Dr. Edelman is clear from the start that he is not trying to take away your sweets; instead, he hopes to educate how you choose your foods so that if you choose to enjoy a dessert or sweet treat, your body hasn’t already been inundated with excessive sugar.

Too much sugar has been linked to obesity, premature aging, heart disease, and other serious conditions. Sugar is also an addictive substance, so the more sugar you consume, the more sugar your body craves. Dr. Edelman hopes that by shedding light on the amount of sugar we consume, we can minimize our dependency on sugar and lead healthier lifestyles.

MSCI’s Dr. Edelman Publishes New Book

We’re proud to announce that Dr. Edelman, of the oncology department at Medical and Surgical Clinic of Irving, has just published a new book. Entitled Sugar Rehab, the book explores the danger of sugar and the effects it can have on the human body.

Edelman’s specific interest in sugar is how it contributes to diseases, including cancer which he combats directly as an oncologist. While we might think of sugar as posing a risk for obesity and diabetes, it can also damage the body in other ways. For example, sugar actually damages the cells of the body in addition to spiking insulin levels. And the latest studies indicate that excessive sugar intake might actually contribute to dangerous diseases, including cancer.

Edelman wrote the book to be a wake-up call and warn people about the potential risks of sugar intake. While a single sweet treat might not be dangerous, sugar sneaks into our diets in other ways—bread, alcohol, processed food, and carbohydrates. Even people who avoid sweets might be consuming too much sugar. Edelman hopes that by reading his book, people will be more aware of their sugar intake and discover a way to live a healthier, potentially disease-free life.

Sugar Rehab is available online as an eBook at There, you can also learn more about the book, Dr. Edelman, or schedule a consultation. You can also download the first 60 pages free on the website.
